If you’re new to our community or just new to our district, welcome!

Enrolling your students in our schools begins with registering an account in our student information system. Once you have registered, you’ll receive a message explaining how to enroll your student(s) in our schools. Our system also uses your home address to determine which school your student(s) will attend. You will receive this information as soon as your registration is complete.

High School - Returning Students

Permission for Medication Form
- Required for the school nurse to administer prescribed or over-the-counter medication
Pre-Participation Physical Evaluation, KSHSAA Concussion Release Form, Parent/Guardian Consent and Insurance Group, Athletics Permission Form
- Required for students to play or plan to play any school athletics
Medical Statement to Request School Meal Modification

Health Assessment for Children and Youth, Kansas Certification of Immunizations(KCI)
